Missing Issues Claims
All missing issue claims will be honored free of charge according to the following guidelines:

Claims for all subscription orders must be submitted within 3 months of the issue date.
Claims submitted after these periods but within the calendar year must be prepaid at the single issue rate. No exceptions.

When submitting a claim, please include:

• Year of publication, volume, and issue.
• Complete ship-to address
• Subscription number found on delivery label
• Agency reference number
• Complete payment information (ie, method, date, and amount remitted)
• Nature of claim (ie, missing issue or receipt of damaged issue)

Submit claims to the corresponding Customer Service Center.

Back Issues
The American Medical Association maintains a total of 13 months of journals. All available journals published in 2007 are subject to the following back issue prices.

Issues older than 13 months may be purchased from the following back issue dealer:

Change of Address
Address changes can be submitted to the corresponding Customer Service Center via mail, e-mail, or fax. We request 4 to 6 weeks advance notice for uninterrupted service. Please provide subscription account number, complete old address information, and new address information.

Subscription Account Numbers can be found on your mailing label, as shown in green on the sample label below.

Online Access
Subscribers will need their subscription account number for a one-time only registration in order to activate online access. Information about activating your online access

Refund Policy
Subscriptions may be cancelled at any time. A prorated refund will be issued.
